This study examines how national health policy aims are turned into practical healthcare services in rural areas. Based on ethnographic fieldwork at an intermunicipal emergency clinic in northern Norway, the study shows how the introduction of municipal acute inpatient care (MipAC) relied heavily on nurses’ everyday work to make the service function in practice.

In a context characterised by long distances, fragmented services and limited medical presence, nurses had to continuously coordinate tasks, adapt guidelines, clarify responsibilities and maintain continuity across services.

The study shows that this organising work was not optional, but necessary. When national health policy aims were delegated to municipalities without clear organisational frameworks, responsibilities and support systems, key issues had to be resolved in everyday practice. Nurses became central in this work not because they actively sought this responsibility, but because their continuous presence and position within the service placed them where gaps, uncertainty and coordination needs emerged.

The findings provide new insight into how health policy is made workable in everyday practice, and how rural conditions can intensify organisational demands in healthcare services. The study contributes to a better understanding of what is required for new services to function over time, particularly in areas characterised by limited resources and long geographical distances.
