Venous thromboembolism (VTE), which includes deep vein thrombosis (DVT) and pulmonary embolism is a life-threatening condition caused by blood clots. P-selectin, a protein involved in inflammation and clotting, is found in plasma both in a soluble form and on the surface of extracellular vesicles (EVs). This thesis investigated whether P-selectin could improve DVT diagnosis or predict incident VTE and recurrence.
In patients with suspected DVT, EV-bound P-selectin showed no diagnostic value, and soluble P-selectin performed only moderately and failed to improve the current diagnostic algorithm. In a population-based study, higher P-selectin levels were associated with an increased risk of future VTE in women but not in men. In another population-based study on patients who already had VTE, P-selectin levels did not predict recurrence. These findings underscore the difficulty of identifying superior biomarkers for VTE and provide critical directions for future research into sex-specific risk factors.
